Your first visit to Elmvale Dental Care lays the foundation for your dental care journey. We'll review your medical history, introduce you to your dentist, and provide all the practical details you need. Save time by completing patient forms in advance—we're here to ensure a smooth, welcoming experience!
Before Your First Visit

Please complete your Medical History and Past Dental History forms online.

If you have previous X-rays you'd like us to review, you can request and complete the X-Ray Release form after your appointment.

For dental insurance, bring the following details: Insurance Company Name, Subscriber Name and Birthdate, Policy/Plan/Group #, and Certificate/ID #.

If you take prescription medications, bring a list of them with you.

When You Arrive
Kathy or Emilie will warmly welcome you at our reception area. If you haven’t completed the forms online, don’t worry—paper copies will be available for you to fill out on-site.


New Patient Examination

One of our dental assistants will seat you in our treatment area

She will ask you some preliminary questions regarding your dental concerns and dental x-rays will be taken for a comprehensive review of different diseases, as needed

Intra-oral photographs will be captured of your teeth and tissues, as needed

Dr. Bodea or Dr. Norsen will review your medical history and do a thorough evaluation of the condition of your teeth, soft tissues, examining for pathology and dental disease, as well as evaluating your bite, gums, and your TMJ. Your x-rays will be further interpreted for a complete diagnosis

Dr. Bodea or Dr. Norsen will explain all the critical details collected from your x-rays, photos, and clinical exam and will summarize in a priority sequence how to best address these findings

Treatment options will be given as well as an explanation of risks and benefits of each treatment, as needed

Sedation options will also be reviewed with you if this is necessary for the treatment to be completed safely and comfortably for you

A predetermination to insurance or fee estimate will be generated by either Kathy or Emile and will be reviewed with you at the end of your visit
First Dental Cleaning

Please tell the dental hygienist any concerns you may have in your mouth (i.e. any sensitive areas)

Please share your previous experiences with dental cleanings and your home care routine

X-rays may be prescribed to assist in the diagnosis of periodontal bone loss and gum disease
-
Comprehensive periodontal probing may be done to objectively measure the depth of your gum pockets and amount of gum recession to customize the right treatment plan for you

The hygienist will review with you any significant areas of concern, oral hygiene care and techniques that are best suited for your needs after your cleaning is complete

Cancellation Policy

Our goal is to provide you with the best possible experience on each and every visit to our office. To achieve this, we spend a lot of time preparing for your appointment and ensuring we have the appropriate staff available for you. Your appointment time is reserved just for you and your provider.
Appointment Reminders
Appointment reminders are sent via text and email 3 days prior to your appointment, which requires a confirmation answer back from you. If confirmation is not verified via text, email, or phone, we will call you 1 day prior to the appointment.
Please be sure to confirm the appointment by replying promptly to the text or email or by calling us at (705) 322-1144 at least 48 hours prior to your appointment
Changes or Cancellations
If for any reason you need to change or cancel your appointment, you must notify us at least 48 hours prior to your scheduled appointment time. Any changes made with less than 48 hours notice and/or no shows may result in a $50 fee.
Late Fee
​Additionally, if you arrive more than 10 minutes late to your scheduled appointment time, we will do our best to accommodate you, but will likely have to reschedule the appointment and the $50 late fee may apply.
Appointment Confirmation
​All appointments must be confirmed 48 hours in advance or are subject to cancellation. We will attempt to contact you by phone, text, and/or email. If we are unsuccessful in obtaining a confirmation of your appointment, it will be removed from our schedule so we can see another patient in need, who has been waiting for an appointment.
